massive cerebral haemorrhage ICD-10: I61
Mann, 73 J., gleicher Patient wie bei Präparat A
III 37. Der Patient hat nach Eintritt der Massenblutung noch 36
Stunden gelebt. Autopsiepräparat
- Ausgedehnte, frische Blutmassen (Blutung) in grauer und weisser Substanz
- Praktisch keine entzündliche Reaktion im umgebenden Parenchym
- Deutliches perifokales Ödem
Makroskopie:
Grosshirnblutung mit Einbruch in den linken Seitenventrikel
73-year-old male patient suffered from essential hypertension. He had acute intracerebral
bleeding to which he succumbed after 36 hours (specimen from autopsy; see also slide A III 37).
- A section of the cortex is observed with an adjacent area of destroyed brain tissue (Blutung). The latter region corresponds to an organized hemorrhage. At the
periphery of the destroyed tissue, numerous lipid laden macrophages are observed. In addition, hemosiderin deposits are focally present.
- The center shows necrotic tissue and lysed erythrocytes.
- The brain tissue immediately adjacent to the hemorrhage exhibits a perifocal edema (perifokales Ödem), marked by a spongy appearance of the tissue.
Macroscopy: The cerebral hemisphere is disrupted and distended by a hemorrhage. The hemorrhage has ruptured into the ventricular system.
